Payment terms

 

Our standard payment term is that payment in full is taken at the time of treatment by the means of cash or via the payment terminal provided by the Clinic.

                                    

Cancellations / Non-attendance

Should circumstances mean that you have to cancel your booking we ask for 24 hours’ notice to cancel the appointment.  In the absence of 24 hours’ notice, or in the case of a ‘no-show’ or late arrival, the full fee for the scheduled service will be payable.

 

Unforeseen circumstances

​

On occasion, unforeseen circumstances may require us to cancel a booking.  In such circumstances you will be given as much notice as possible.

 

Therapy Fees are available on request or as per our website.  Fees are subject to change without prejudice.

 

Location of Clinic

 

We provide therapies at the location detailed on our website.  We reserve the right to change location if necessary.

 

Therapists

​

Darren Worsfold is the Director of DKW Sports Therapy Ltd.  Other therapists may provide therapy treatments but are not servant of DKW Sports Therapy Ltd and provide services as an independent contractor.

 

All therapists maintain adequate professional indemnity insurance and are qualified to provide the therapy services.

 

The customer undertakes the therapy at their own risk.

If we ask for personal information, we will:

• only collect personal data about you to enable us to provide you with our services only use the information you provide to help us provide this service take treatment notes following each session

• only use your data for the purpose for which it was collected and never share it with any third parties without your prior consent use your data to manage future communications between us

Retention and Storage of Data

Data is held in secure computer and manual files.We have approved measures in place to stop unlawful access and disclosure

We will retain your information for 7 years.Once the retention period has passed, all information will be removed from our systems and manual files securely destroyed
